There is a Bookings Clerk who will manage your admission to the day surgery. The doctor will give you a blue and white form to complete and this includes your consent to the procedure, your medical & medication history and your privacy consent. This form must be completed and returned to the day surgery before an appointment can be booked. Once this is received your admission can begin.
